On behalf of our entire staff, I want to thank you for contacting us and welcome you to our office. We want you to know that we will do everything in our power to exceed your expectations and provide you and your family with the best care anywhere. What makes us unique, and I feel special, is our mission statement. Please take a minute to read it because it truly reflects our treatment philosophy and what you can expect from us.

Your first visit is very important. You can anticipate being here for about an hour. During this time you will be busy with a lot of record taking and other data collection including the best, most comprehensive examination that you have ever received. We do a complete head and neck exam that includes looking at your joints, muscles, soft tissues, and an oral cancer screening. Next we look at the health, function, and appearance of your teeth and any existing restorations. Finally we look at your gums to evaluate periodontal health. Our goal is to fully understand you and your mouth so that during the course of our conversations we will be able to provide you with all the information you need to decide a course of treatment that best suits you. At your second visit we clean your teeth and discuss any relevant findings. Sometimes we may decide to schedule a completely separate consultation visit, at no additional charge, to talk about more complex problems and solutions.

We have developed our own forms and have included them in this packet for you to fill out prior to your appointment. Please fax them back or bring them with you. The information we gather will help us serve you better. Our goal is to make you so happy that without hesitation you will enthusiastically refer others to be treated the same way. If at any time you have any questions or concerns, please do not hesitate to call them to the attention of any member of our team. We will be sure to address them promptly and to your complete satisfaction.
